To request relief services, please fill out the request form at the bottom of this page or email Dr. Weaver.

Please include the following in your request:
1. Requested date(s)
2. Requested contracted hours
3. Requested service type (exams/surgery/urgent care/walk-in clinic).

Confirmation is sent out via google calendar invitation and email no later than 1 month prior (though dates are typically booked on a quarterly basis) to the confirmed date.

Days marked “busy” are booked or unavailable.

You may still submit requests for days marked “busy”, these will be added to a request list and may be filled in the event of a cancellation.

Confirmation of requested dates are sent as soon as possible, but no later than 1 month prior to the date requested.
