To request relief services, please fill out the request form at the bottom of this page or email Dr. Weaver.

Please include the following in your request:
1. Requested date(s)
2. Requested contracted hours
3. Requested service type (exams/surgery/urgent care/walk-in clinic).

Confirmation is sent out via google calendar invitation and email no later than 1 month prior to the confirmed date.

The calendar below shows availability and is updated as dates are booked.

Days marked “busy” are booked or unavailable.

You may still submit requests for days marked “busy”, these will be added to a request list and may be filled in the event of a cancellation.

Confirmation of requested dates are sent as soon as possible, but no later than 1 month prior to the date requested.
