Scheduling for Existing Clients:
To request relief services, please fill out the request form at the bottom of this page or email Dr. Weaver.
Please include the following in your request:
1. Requested date(s)
2. Requested contracted hours
3. Requested service type (exams/surgery/urgent care/walk-in clinic).
A google calendar below shows days that are already booked/unavailable marked as Busy.
Requests may be made as far in advance as you would like.
Confirmation is sent out via google calendar invitation once approved and via email 1-2 months prior to the confirmed date.
